Sponsor to use name on his CMI account, either the chosen personal name or company name, with
proper ssn.  How a person named his account must be used on all forms.

=Mailing Address Notice
CMI uses the Post Office to ship the coins.   We recommend that you verify your mailing address.  Go
to www.usps.com, use Zip Code search, and validate the format of your address there.
You can submit only ONE address to CMI. This single address will receive your coins, news, and letters
from CMI.  Sorry, we can maintain only one address.

All blanks filled out, signatures and dates-- front and BACK!

If bank drafting:  
Fill out members CMI Account name, ID number, and checking account name.
SIGN it.
Provide complete bank info.
FILL IN WHAT MONTH TO START THE DRAFT,
for example, if a member joins in Feb, the first draft date will be March.
[UFP drafts on the 20th; UBP, Stage1&2 Masters drafts on the 25th]  

Fill in the amount of purchase. There are 4 parts:  the coins, the subtotal, the draft fee appropriate to
the subtotal, and the grand total that will drafted.  
Attach a voided check.  It is OK to submit a starter check. Please write the name of the account if not
already preprinted.
WE DRAFT FROM ONLY CHECKING ACCOUNTS. Also, if you send anything other than a voided check, ie
a deposit slip or handwritten information, we do not accept that format.  We will accept letters from
your bank.  Sorry for the inconvenience, but if the information is incorrect and the draft is rejected,
there will be a fee charged to you.
If you do not pay online electronically, the mailed in payment must be MONEY ORDERS AND CASHIER’S
CHECKS MADE OUT TO CMI. No personal or business checks. No checks made out to anyone or
anything other than CMI. We accept only coin orders. We do not process sales kit orders or anything
related to sales. Please contact headquarters in MO to request that material. Do not add it onto your
coin orders.

ALSO, the bank draft fee must be added,  for those wishing to start the automatic withdrawal.

=Do not use staples. Paperclips only, or no attachments at all.

=Cut off date. Please send in your paperwork to be received by the 25th of the month. On the 1st of
the next month is when we post all payments.  LATE ARRIVALS WILL BE IN THE NEXT MONTH’S
POSTING.
